NHS Diabetes Prevention Programme Return on Investment Tool V1.0

An adaptation of the School for Public Health Research (SPHR) diabetes prevention model, which was developed with funding from the National Institute for Health Research (NIHR)


The NHS DPP Return on Investment Tool will help you to understand the costs, savings and health benefits
likely to be produced by implementing the DPP in your local authority or clinical commissioning group.


The Healthier You NHS Diabetes Prevention Programme (NHS DPP) is a joint endeavour from NHS England, Public Health England and Diabetes UK to deliver an evidence-based intensive lifestyle intervention to individuals at high risk of type 2 diabetes due to diabetic hyperglycaemia (HbA1c 6.0-6.4%). The programme aims to improve health, prevent diabetes and reduce healthcare expenditure, and will be rolled out across England from 2016.

This tool simulates what will happen in your local area if the NHS DPP is implemented for one year. Your local population is simulated based on demographic characteristics; age, sex, ethnicity, deprivation and diabetes prevalence. The population is then simulated for 20 years after the NHS DPP to estimate the effects on their health, changes in healthcare costs and to calculate the return on investment.

The default settings are based on Public Health England and NHS England's current estimates of the cost and uptake of the intervention at a national level. You can change all the inputs to reflect your local experience, or to estimate different scenarios. The default levels of effectiveness (weight loss and reductions in other risk factors) and time over which people regain weight and return to the risk they would have had without the NHS DPP (time to return to baseline) are based on the best available evidence to date on expected outcomes. For the effectiveness of the intervention there are three options; a central estimate, based on PHE’s recent evidence review and a lower and upper bound with 25% lower and 25% higher effectiveness levels.

The tool will create an online report summarising the results for your area, given your specified inputs. A downloadable PDF report and results file will also be available.
